Job Description

As a member of an Assertive Community Treatment (ACT) team, functions as a case manager with primary responsibility for providing services, implementing plans for treatment, rehabilitation and support services. Coordinates, monitors, and provides direct services to members in a community setting. Participates in the development of individualized service plans (ISPs) and provides/coordinates services related to ISPs. Acts as a member advocate, coordinates services and discharge plans, and assists in crisis intervention. Provides after-hours care, on-call coverage, and support to all members being served by the ACT team.

All members of our team are expected to provide services in accordance with the Nine Guiding Principles for Recovery Oriented Adult Behavioral Health Services and Systems.

REQUIREMENTS

Education, Certification, and Experience Requirements
  • Must possess one of the following:
    • Master's or bachelor's degree with a behavioral health or related field
    • Bachelor's degree in a non-related field PLUS one year related experience
    • Associate's degree in any field PLUS one year related experience
    • High school diploma or equivalency certificate PLUS three years' related experience
  • Minimum (1) one year working with seriously mentally ill adults required; previous experience working on an ACT team or equivalent experience preferred.
  • Must maintain current CPR, First Aid, and Therapeutic Options certifications (offered through Southwest Network)
  • Must maintain a valid Arizona driver's license and car insurance
  • Must be at least 21 years of age.
Knowledge, Ability, and Skills
  • Must have the ability to maintain excellent member and family relationships and provide excellent customer service
  • Requires effective written, verbal and interpersonal communication skills
  • Ability to stay organized and have critical thinking, time management and problem solving skills are a must
  • Ability to utilize multiple types of office equipment (i.e. phones, copier, fax, etc.)
  • Ability to type 25 to 40 WPM is preferred
  • Must be proficient in Microsoft Office software
  • Must be able to utilize the internet and various web browsers

Our Assertive Community Treatment (ACT) program focuses on providing intense, frequent, face-to-face services to members with persistent and severe mental illness and/or substance use disorders. This evidence-based practice helps improve outcomes for people who are most at-risk of homelessness, psychiatric hospitalization, and institutional recidivism.
